Use this form if you are a full-fee paying international student who has been studying offshore entirely in Quarter One 2023, Quarter Two 2023 or Semester One 2023 and would like to request an insurance waiver.

Waiver applications must be received on or before the following date and any waiver application received after this date will not be accepted:
