ISS-02 International Insurance Waiver Form for Incoming Study Abroad and Exchange

Use this form if you are an Incoming Study Abroad or Exchange international student and would like to request an insurance waiver.

Please note: Your alternative compliant insurance policy must cover you up until the day your student visa expires or until you depart New Zealand, whichever comes first.

Waiver applications must be received on or before the following dates and any waiver application received after these dates will not be accepted:

Summer School 17 January 2020
Semester One 13 March 2020
Semester Two 7 August 2020

If you have an approved policy stated in the compliant policy list, then you MUST provide the following documents:

Insurance certificate in English
Itinerary - return ticket home showing your departure (date must be before visa and policy expiry date)
Visa - copy of your student visa

Your applications will not be accepted if you do not meet all the requirements and do not have all the correct documents.

Student declaration

I declare that while I am a student of the University of Auckland, I will have an approved alternative insurance policy for the period of my enrolment and student visa, in accordance with Immigration NZ regulations. I am required to inform the International Student Adviser of any changes during the insurance period (eg, I am no longer enrolled, I have a change of status to domestic student or I enrol with a different provider) OR if I am studying overseas or conducting research overseas, on suspension or on student exchange before waiver due dates.

I agree to apply for an insurance waiver for every quarter or semester I am enrolled in (for doctoral students every year). If not, the University will charge my customer account for their preferred insurance policy (Studentsafe-University). The University of Auckland is not liable for any claims not met by any insurance assessed as compliant or not compliant to the Code of Practice for the Pastoral Care of International Students.
