Request for digital copy of graduation certificate(s)

This form is to request a digital certificate for Graduates prior to 2010. Your certificate will be issued on the My eQuals platform.

To access My eQuals you will need your University username and password. You can reset your password (if needed) via Change or reset password. Your password for My eQuals is the same as your University password. You may also need to set up 2 Factor Authentication as many University services now require this.

My eQuals enables faster sharing of your transcripts, certificates or official letters online, by simply sharing a link to your document.
